1. Information we collect

When you contact us by phone, email, or in person, we collect the information you choose to share — typically your name, contact information, and the reason for your visit. When you become a patient, we collect health information necessary to provide care, in accordance with HIPAA.

2. How we use information

Patient information is used to provide care, coordinate with your other providers when authorized, and bill insurance or Workers' Comp carriers. We do not sell or rent patient information.

3. HIPAA notice (summary)

As a healthcare provider, we are a HIPAA covered entity. We follow HIPAA Privacy and Security Rule requirements for protected health information (PHI). For the full Notice of Privacy Practices, including your rights under HIPAA, see Notice of Privacy Practices or request a copy at the clinic.

4. Email security

Standard email is not encrypted and is not a HIPAA-secure channel. Please do not include sensitive medical information (symptoms, diagnoses, treatment history) in email. For clinical questions, call (909) 239-6094.

5. Cookies and analytics

This site uses Vercel Analytics for aggregate, privacy-friendly traffic data. When configured, it may also use Google Analytics 4 to measure how visitors interact with the site (page views, phone-link clicks). These tools may set cookies or similar identifiers. No protected health information is collected through analytics.

6. Third-party services

The site embeds Google Maps to show our location and links to Google for reviews. Use of those services is governed by Google's privacy policy. The site is hosted on Vercel, which processes connection metadata as part of operating the service.

7. Patient rights

Under HIPAA, you have the right to access your records, request amendments, request restrictions on certain uses, and receive an accounting of certain disclosures. See the Notice of Privacy Practices for details and how to exercise these rights.

8. California residents (CCPA)

We do not sell or share personal information for cross-context behavioral advertising. California residents have the right to know what personal information we collect, request deletion, and not be discriminated against for exercising these rights.
