Our commitment

Acuzen Inc. (doing business as S. Kim Acupuncture & Herb) is a HIPAA covered entity. We are required by law to maintain the privacy of your protected health information (PHI), provide you with this notice describing our practices, and follow the terms of the notice currently in effect.

How we may use and disclose your information

We use and disclose your PHI for treatment, payment, and healthcare operations. Examples include sharing records with other providers involved in your care, billing insurance or Workers' Comp carriers, and internal quality review. Other uses and disclosures will be made only with your written authorization, which you may revoke at any time.

Your rights

• Inspect and obtain a copy of your records, in the format you reasonably request.
• Request an amendment to information you believe is incorrect.
• Request restrictions on certain uses or disclosures of your information.
• Request confidential communications by alternative means or at alternative locations.
• Receive an accounting of certain disclosures we have made of your information.
• Receive a paper copy of this notice on request.
• File a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.
